I HEREBY ACKNOWLEDGE that I am present with the minor in question, and I will ensure that they will adhere to the AFTERCARE ADVICE which will be received both verbally and in written format at the end of the piercing procedure. Failure to follow this advice could lead to the piercing becoming infected.
